Group Adult Foster Care (GAFC) is a program designed for elderly or disabled individuals who need assistance with activities of daily living through regular caregiver visits.

What is Group Adult Foster Care?

Group Adult Foster Care (GAFC) helps older adults and people with disabilities stay safe and healthy in their own homes. Care is provided by a caregiver, often a family member or friend, who visits every day or on a regular basis to help with bathing, dressing, meals, and other daily tasks. Caregivers receive support from VITRA Health.

Each client is assigned a nurse who helps create a care plan based on their needs. A case manager works with the client and caregiver, helps connect them to services, and answers questions along the way. Bilingual services are available.
