Family Services

The purpose of Family Services is to provide custom care, resources and medical oversight for our Adult Family Care and Visiting Adult Family Care clients.

Family Services Roles
RN/Case Manager

Conduct regular in-person visits with client and caregiver to ensure clients remain safe and healthy in the community while decreasing/preventing unnecessary hospitalizations and re-hospitalizations.

  • Ongoing evaluation of client’s health status, including formal semi-annual and annual assessments
  • Complete required assessment documentations
  • Contact with client’s primary care physician to obtain medical information and physician clearance; also reporting any changes in medical status
Social Services Case Manager

Plan, implement, monitor, document, and evaluate actions required in meeting the client’s health and human services needs.

  • Maintain and update client charts and medical records
  • Help patients make informed decisions by acting as their advocate
  • Collaborate with the medical team throughout the entire case management process

Caregiver Services

Caregiver Services consists of individuals who visit and provide clients of the Group Adult Community Care program with assistance based around their daily needs.

Caregiver Services Roles

Assist with Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs) including skilled tasks when applicable and offer companionship to clients.

  • Provide medication reminders
  • Keeping notes and reporting any changes in health to your supervisor
  • Complete necessary documentation
Home Health Aide / Certified Nursing Assistant

Provide care under the supervision of a Registered Nurse (RN) and offer companionship to clients.

  • Provide medication reminders
  • Keeping notes and reporting any changes in health to your supervisor
  • Complete necessary documentation

Service clients who need assistance with Instrumental Activities of Daily Living (IADLs), but do not require assistance with personal care.

  • Instrumental Activities of Daily Living (IADLs) may include: meal preparation, light housekeeping, laundry, running errands, grocery shopping, etc.
  • Offer companionship to the clients
  • Provide medication reminders
  • Complete necessary documentation
Meals On Wheels Driver

Deliver meals to clients on a specific daily route.

  • Must have a valid driver’s license and access to a reliable vehicle
  • Must be able to lift and carry up to 35 pounds and climb stairs multiple times per day
  • Must be able to understand a color-coded system for meal identification
  • Bilingual a plus

Skilled Services

Social Services consists of our Visiting Nurse Agency, which provides clients with skilled nursing, medication management, disease process education, and mental health education.
Skilled Services Roles
Licensed Practical Nurse (LPN)
Support patients with medication management and disease process education. Help clients gain a deeper understanding of their health and gain skills and self-confidence to manage their conditions on their own.

Shared Services

Shared Services consists of our office-based departments such as Compliance, Marketing, Finance, Intake, and Human Resources.

Shared Services Roles
The Finance department acquires and manages funds within the company, as well as plans the expenditure of funds amongst various assets. Finance consists of roles such as Finance Director, Billing Manager, Accounts Payable Specialist and Staff Accountant.
Human Resources
Human Resources finds, screens, recruits and onboards job applicants, as well as administers and manages employee-benefits programs. Human Resources consists of roles such as HR Director, Talent Acquisition Manager, Recruiter, and HR Coordinator.
Information Technology
Information Technology (IT) establishes, monitors and maintains all technological systems and services within the company. IT consists of roles such as IT Manager and IT Specialist.
The Intake department is responsible for the admission process of new patients as well as maintaining relationships with partners such as Primary Care offices and Insurance Companies. Intake consists of roles such as Intake Director, Intake Manager, Intake Case Manager, Intake Account Executive and Insurance Authorization Specialists.
Marketing creates and communicates to engage a target audience and build strong relationships of value to clients and the company alike. This department consists of the roles of Marketing Manager, Outreach and Branding Specialist, Digital and Social Media Specialist, and Internal Marketing Specialist.

Adult Day Health

Adult Day Health is a daily care facility designed to meet the physical, functional, and social needs of elders or individuals with disabilities, including those with complex medical or rehabilitative needs.
Adult Day Health Roles
Activities Coordinator

Assist in the planning and implementation of all activities and therapeutic programs to meet the physical, mental, and psychosocial well-being of each client at our Adult Day Health facility.

  • Assist in developing a written plan of care for each resident’s activity program
  • Develop, implement and post the monthly activity calendar with a creatively attractive monthly theme
  • Produce an online and printable quality monthly newsletter and calendar and daily activity calendar
  • Responsible for leading and implementing one-on-one, small group, and large group programming
Patient Transporter

Provide safe and time-efficient transportation of our Adult Day Health participants to and from our facility in Brockton.

  • Ensure the safety and well being of all clients while in the vehicle
  • Complete all routes as near to schedule as possible
  • Be courteous to all clients, residents, employees and public at all times.
  • Maintain company vehicles
