What is Structured Family Caregiving?

Structured Family Caregiving (SFC) is a program for individuals aged 60 or older who are enrolled in the PASSPORT waiver program and live with a caregiver who provides support with daily needs.

This program supports individuals through visits from trained caregivers, who may be a friend, family member, or VITRA Health staff. Caregivers assist with personal care, supervision, and daily household activities, all while being supported by VITRA Health.

SFC Caregiver Requirements

To be eligible to serve as a SFC caregiver in Ohio, an individual must meet the following qualifications:

  • Be at least 18 years old.
  • The caregiver and individual must live together, either in the individual’s home or in the caregiver’s home.
  • Must be a spouse, parent or legal representative, cannot be a foster parent
  • Caregivers must successfully complete a training and competency evaluation program
