Finding out if you qualify for Adult Foster Care or Group Adult Foster Care can be labor intensive. Follow these simple steps to make the process easier and less challenging.
Step by Step Qualification Process

Step 2 | Schedule a Consultation
We will schedule an assessment with one of our Senior Care Access Navigator to find out how we can help you.

Step 3 | Determine Insurance Coverage
Our team will verify the insurance coverage and work with you to find the best payment option for you.

Step 4 | Medical Needs Assessment
A licensed healthcare professional from our team will assess the needs of the person you care for and begin to develop the plan of care.

Step 5 | Begin Receiving Services
After the primary care physician (PCP), or primary care provider and insurance company approve, our team will start providing you with the support you need to take care of your loved one.

Client/Member Eligibility
For Adult Foster Care, the person receiving care must be at least 16 years of age
For Group Adult Foster Care, the person receiving care must be at least 22 years of age
Needs daily cueing, assistance, or supervision of ONE or more Activities of Daily Living (Group Adult Foster Care)
Needs 24-hour cueing, assistance, or supervision of ONE or more Activities of Daily Living (Adult Foster Care)
Must be under the care of a Primary Care Provider
Program participation must be ordered by a Primary Care Provider
Must live in a safe home environment
Must have MassHealth Standard (Medicaid), MassHealth CommonHealth, Senior Care Options
Caregiver Eligibility
Must be at least 18 years of age
Can be related to the person needing care or a friend, but cannot be the legal spouse or guardian
For Adult Foster Care, must live with the member requiring care
For Group Adult Foster Care, must be able to travel to the member requiring care
Can spend time away from home or working if there is an informal support
Must be invested and understand the needs, health, and wellness of the member
Must pass a CORI check
Must pass a physical examination by a medial doctor to determine caregiver eligibility
