Steps to Approval

We guide you through every step to make the approval process as smooth as possible. Here’s what to expect:

1. Phone Interview to Start the Admission Process
   • A care access team member will call to complete your application and answer all your questions.
   • They’ll request copies of your photo ID, proof of address, and assist with signing a Release of Information for your primary care provider.
   • You or your caregiver will need access to email.
2. In-Person Assessment by a Registered Nurse and Case Manager
   • After receiving notes from your primary care provider, the care access team will schedule a home visit.
   • The registered nurse will assess if you qualify for the program through a conversation-based evaluation.
   • The case manager will ensure the home is safe, clean, and equipped with safety tools like a first aid kit, fire extinguisher, and smoke/CO2 alarms.
3. Request Medical Documentation
   • The care access team will contact your primary care provider to collect necessary documents, including a medication list, diagnosis list, and recent office visit notes.
   • If you haven’t had a physical in the last 12 months, you’ll need to schedule one with your PCP.
4. Submit to Insurance for Authorization
   • The care access team will send the registered nurse’s assessment and your primary care provider’s order to the insurance company for approval.
5. Admission Visit and Custom Care Plan Creation
   • Once approved, the care access team will schedule an admission visit with the registered nurse and case manager.
   • You’ll sign an agreement with VITRA, and daily care will begin right away.

By following these steps, we ensure a smooth transition into the program, providing caregivers and clients with the support they need.
