Group Adult Foster Care (GAFC) is a program designed for elderly or disabled individuals who need assistance with activities of daily living through regular caregiver visits.

Caregivers assist with personal care, supervision, and daily household activities, all while being supported by VITRA Health.
Each client is assigned a Registered Nurse to oversee their needs and develop personalized care plans. A Case Manager provides support for caregivers, coordinates with medical and insurance personnel, advocates when needed, and connects families with resources. Bilingual services are also available.
Clients receive support with activities like bathing, dressing, mobility, and other daily needs.
Caregivers visit daily to assist with tasks like grocery shopping, laundry, meal preparation, homemaking, medication reminders, and more.
