Keeping Individuals Connected with the Help of Case Management Expertise

The Social Services department at Vitra Health is a team of case managers who work collaboratively with other Vitra services and support to provide the highest quality of care to the caregiver and clients of Vitra Health. The case managers provide education on health and wellness, advocacy and support between caregivers, clients and providers. Additionally, case managers identify and provide caregivers and clients with important resources in the community. Case managers provide these services through weekly calls to caregivers and visits with the caregiver and client.
